(*) - Late registration: All applications postmarked after Sept. 10, 2004 must include the late registration fee as shown above. Late registration is available beginning at 7:00 a.m. on the day of the conference. All mailed registrations must be received on or before Sept. 15, 2004. Seating for late registration may be limited.
Refund Policy: Requests must be received in writing by Sept. 17, 2004. A withdrawl fee of $50.00 will apply. Substitutions are permitted.
